Ship Theseus 0.0.1 rebuild: new hashes + deploy the restructured site

- Theseus installer/portable rebuilt with the toolbar overhaul; updated
  SHA-256 in releases-manifest.json, releases/index.html, apps/index.html
  (Setup 91764aba…, portable 41a54c7e…). Binaries live on dl.silentmode.st;
  restructured site (shared.css, apps/nav/store/hermes/choose) deployed to
  Sia (silentmode.bch).
- bns-indexer: main-module detection via pathToFileURL.
