Pithos 0.3.14: accounts at the top, Cancel works, updates without a restart

The Accounts page opens with every Sia account on this computer side by
side, with Switch to and Add another account; Cancel closes dialogs
again and Escape works after a ⋯ menu; the footer's update applies the
new version in place on a Theseus that supports it.
